Thursday 20/05/04
George & Doreen (Middleton) visited Denis am
Took Denis to collect his pension and to pay his rent
**
Mel & June visited evening
Denis VERY unsteady on his feet and fell in bedroom
(nothing to trip over)
Susan is arranging use of Access Bus.
(to and from Morrisons Supermarket)
** June & Mel to set up direct debits for bills and direct payment of state pension into Denis's bank account.
Wednesday 19/05/04
Denis home
- June & Mel sorted his shopping requirements.
Denis is ok mentally but VERY UNSTEADY when walking !
Walk-in shower to be
requested (via Doctor).
Walk-in shower installed NOVEMBER 2005 (18 MONTHS !!)
Access bus to Morrisons supermarket being organized 
Monday 17/05/04
June & Mel visited - Denis now walking better (but still unsteady)
Consultant to visit tomorrow & discuss home help requirements etc.
Saturday 15/05/04
Denis transferred to Ward 11 -  Chapel Allerton Hospital (Leeds).
He is improving daily and
will remain at Chapel Allerton until home care is in place (possibly Tuesday or Wednesday).
June & Mel visited Chapel Allerton (Ward 11) Saturday pm

The results of the scan were ok but other tests had shown an infection in his water - this probably caused the problems and has now been treated and cleared.
We did not see Denis walk today but otherwise he definitely seemed 'back to normal'.
Thursday 13/05/04
George & Doreen (Middleton) visited Denis (am)

June & Mel (pm) - results of scan received.
ok (other than normal ageing ??)
Denis had made a cup of tea in 'a hospital kitchen' - under supervision - and MAY be allowed to go home on Monday (17/05/04).
Denis still has periods of confusion and has no recollection of events on Monday that resulted in his admission to hospital !
He walks but only with assistance and is still VERY unsteady on his feet.

Wednesday 12/05/04
Noreen & Terry visited Denis - still in bed, but otherwise seemed fine
Tuesday 11/05/04
June & Mel visited -  results of scan not yet received.
Should be available by Thursday.
Denis will have to undergo supervised tests in a kitchen to assess his needs.
He will not be able to leave hospital until AT LEAST Monday 17th
Sunday 09/05/04
June & Mel visited - Denis improved (mentally) but no recollection of Monday.
Denis thought he was admitted to hospital on Tuesday

June & Mel visited - further improvement
Purchased a battery shaver for Denis
Friday 07/05/04
Scan Friday am
George & Doreen (Middleton) visited Denis

June & Mel visited  - Denis improved
June & Mel saw Doctor Turner - several blood sample taken
Wednesday 05/05/04
Denis transferred from Ward 44 to Ward 27  (old A&E building)
Tuesday 04/05/04
June & Mel saw Doctor Collins - further tests required.
Denis very confused

Monday 03/05/04  Call from Susan Holmes (Warden)  
Denis ‘buzzed’ her at 4:45pm
Denis was confused – someone (or a couple ?) in flat & dinner (frozen ?) stolen ???
Emergency doctor called and Denis taken to LGI
Leeds General Infirmary  (0113) 2 432 799  -  visiting 8.00am to 8.00pm  
Initially Ward 44 (Admissions Ward)
Ward 27 - Martin Wing
old Accident & Emergency entrance - road on left just up past Brotherton Wing Entrance - lift or stairs to D floor & Ward 27
Consultant Dr Turner - Primary Nurse Helen Pease.

Wednesday 28/04/04
Denis fell outside flats  - cut nose and leg
Seemed ok mentally when June & Mel visited as usual on Thursday 29/04/04
